A CLINICAL STUDY ON DOUBLE CONTRAST DIAGNOSIS OF EARLY GASTRIC CARCINOMA

Abstract
"Early cancer of the stomach", being defined as the carcinoma not reached any deeper layer than the mucosa or submucosa of the viscus, has been reported to have remarkably high 5-year survival rate of 93%. To us, this is indeed aneye-opening fact since the gastric carcinomas are collecting the highest toll each year from the population living in this part of the globe.
During the 15 months period of December, 1968 through February, 1970, we have conducted a series of "early cancer" detection studies at the teaching hospital level by adopting "distension" double-contrast roentgen examination of the stomach. Some 2,780 subjects of both sexes, aged 35 years or more, with various upper gastrointestinal complaints have been studied by upper GI series reinforced with double contrast technique at the Department of Radiology, St. Mary¢¥s Hospital, Seoul. The negative contrast medium was CO2 formed in the stomach from an effervescent agent after quenching of bubbles with 0.5% aqueous solution of polyethylene glycol.
As shown in Table I, 673 cases revealed one or more radiologically demonstrable abnormalities in the stomach and/or duodenum including 27 cases of possibly "early cancer". Of these, 5 patients have been operated upon after medical management and follow up study. The histopathological examination of the resected stomach and regional lymphnodes disclosed the tissue findings to be compatible with poorly differentiated "early adenocarcinoma¢¥ of II-c type in 2 patients; carcinoma invaded the muscular layer but with preserved serosa and free regional iymphnodes (Mo type of Masuda) in 1 patient; adenocarcinoma with serosal penetration and involvement of regional lymphnodes in another patient; and chronic gastric ulcer with no evidence of malignancy in the last patient.
Although the number of operated cases is yet small and inadequate to draw valid conclusion, nevertheless the results are well comparable to those of other investigators.
